Job Summary

Vaya Health is seeking a highly motivated and compassionate Care Management Extender to join our team. As a Care Management Extender, you will play a vital role in supporting care managers in delivering tailored care management services to our members.

Key Responsibilities
  • Perform general outreach, engagement, and follow-up with members to ensure they receive the necessary support and resources.
  • Coordinate services and appointments, including appointment reminders and transportation arrangements.
  • Engage in health promotion activities, such as providing education on chronic conditions, teaching self-management skills, and sharing self-help recovery resources.
  • Share information with the care manager and other members of the care team on the member's circumstances.
  • Provide and track referrals, and assist members in obtaining and maintaining community-based resources and social support services.
  • Participate in case conferences and support the care manager in assessing and addressing unmet health-related resource needs.
Requirements
  • High school diploma or GED required.
  • Must be at least 18 years of age and trained in Tailored Care Management.
  • Must meet one of the following requirements: lived experience with an I/DD or TBI, Certified Peer Support Specialist, parent or guardian of an individual with an I/DD or TBI, or two years of paid experience performing similar functions.
Preferred Qualifications
  • Certified PSS (Peer Support Specialist) and/or WRAP (Wellness Recovery Action Plan) Facilitator.
Working Conditions

This is a remote position that requires the ability to work independently and as part of a team. The ideal candidate will have excellent communication and interpersonal skills, and be able to work effectively in a fast-paced environment.
