Caregiver Support Specialist

Position Summary
The Community Health Worker (CHW) plays a vital role in our care team, implementing a person-centered plan of care and conducting regular home visits. This role requires building and preserving trusting relationships within the community, providing general support and encouragement to consumers and caregivers. The CHW will achieve this by developing and implementing health management plans, coaching consumers to effectively manage their chronic health conditions, and promoting self-care.
Key Responsibilities
- Assist Care Managers and Family Specialists with referral applications and home visits
- Collaborate with the care team to implement consumer plans of care
- Identify community resources to assist consumers and caregivers
- Assist consumers in accessing health-related services and overcoming barriers to medical care and social services
- Collect necessary documents and ensure compliance with standard procedures
- Conduct daily reviews of case record documentation and follow up as needed
Requirements
- 1+ years of experience working with elders or people with disabilities
- Community Health Worker (CHW) Accreditation preferred
- Residency within the local community for 2+ years
- Familiarity with community resources
- Proficiency with case management software and mobile apps
- Ability to travel within assigned geographic area
- Bilingual preferred
