Duties and Responsibilities:

  • Collaborate with the In-Home Therapist to develop and implement a treatment plan tailored to the youth's needs.
  • Provide phone contact and support to the youth and family, promoting healthy functioning and emotional well-being.
  • Work with the In-Home Therapist to provide family therapy, addressing emotional and mental health needs.
  • Identify and connect the youth and family with community resources, promoting sustainability and support.
  • Develop and review risk management and safety plans in collaboration with the youth and caregiver.

Requirements:

  • Minimum 1 year of experience in a human services field.
  • Bachelor's Degree in a human services field, with MCE approval.
  • Bilingual candidates are preferred, with Spanish/English proficiency.
