Speech Language Pathologist

We are seeking a skilled Speech Language Pathologist (SLP) to join our team in a skilled nursing facility setting. As an SLP, you will be responsible for evaluating and assessing speech, language, cognitive-communication, and swallowing disorders in residents. You will develop and implement individualized treatment plans to address communication and swallowing deficits, enhancing residents' quality of life.
Key Responsibilities- Evaluate and assess speech, language, cognitive-communication, and swallowing disorders in residents.
- Develop and implement individualized treatment plans to address communication and swallowing deficits.
- Provide direct therapy services, including individual and group sessions, to improve speech, language, and swallowing functions.
- Educate residents, families, and caregivers on strategies and techniques to support communication and swallowing needs at home and in the facility.
- Collaborate with interdisciplinary teams to ensure comprehensive care for residents.
- Conduct regular progress assessments and adjust treatment plans as necessary.
- Document evaluations, treatment plans, progress notes, and discharge summaries in compliance with healthcare regulations and facility policies.
- Master's degree in Speech-Language Pathology from an accredited program.
- Current state licensure or certification as a Speech-Language Pathologist (SLP).
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) is preferred.
- A minimum of 1-2 years of experience in a skilled nursing facility or geriatric setting.
- Strong knowledge of speech, language, and swallowing disorders, particularly in the elderly population.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of an interdisciplinary team.
- Strong organizational skills and attention to detail.
- Compassionate and patient-focused approach to care.
- Proficiency in electronic health records (EHR) systems.
