As a LPN Private Duty Nursing Specialist, you will be responsible for delivering exceptional patient care, working collaboratively with our team to ensure the best possible outcomes.
Key Responsibilities:
- Utilize the nursing process to assess, plan, implement, and evaluate patient care, ensuring the highest level of quality and safety.
- Provide skilled nursing care and services in accordance with prescribed orders, working closely with healthcare professionals to ensure seamless care transitions.
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition, collecting and analyzing data to inform care decisions.
- Collaborate with the healthcare team to develop and implement individualized care plans, prioritizing patient needs and preferences.
Requirements:
- Current licensure as an LPN/LVN in the state where you will practice.
- Current TB or Chest X-Ray, and current BLS card.
