Job Summary:

This is a local contract position for a Home Health LPN / LVN. The successful candidate will have a strong background in home health care, with a focus on providing compassionate and individualized care to patients in their homes.

Responsibilities:

  • Develop and implement individualized care plans for patients
  • Conduct thorough assessments and evaluations of patients
  • Collaborate with other healthcare professionals to ensure seamless care
Requirements:
  • Active Professional licensure within state
  • 1 year of recent experience within this specialty
  • BLS/ACLS Certification from the American Heart Association
