Job Summary

We are seeking a highly skilled LPN/RN to join our team as an Outpatient Family Medicine Nurse. The ideal candidate will have a strong background in clinical nursing and a passion for delivering high-quality patient care.

Responsibilities
  • Assist with the completion of patient assessments, including medical checklists, physical health screenings, and medication lists
  • Administer medications and observe the Seven Rights of medication administration
  • Provide education to patients and families on disease processes and medication use
  • Monitor patients for potential side effects of medications and report findings to physicians
  • Provide direct patient care, evaluate outcomes, and consult with specialists as needed
  • Accurately assign triage categories and maintain accurate and continued nursing documentation
  • Identify and address psychosocial, cultural, ethnic, and religious/spiritual needs of patients and families
  • Communicate effectively with physicians regarding patient physical and mental conditions
  • Administer medical and psychiatric screenings as needed
  • Perform ancillary testing and tasks as ordered by physicians
  • Supervise nursing functions and ensure conformance with established standards and procedural requirements
  • Assist patients with assistance programs and referrals, and maintain updated files of referrals
  • Respond to consumers' requests for medication refills
  • Assist with meeting reporting requirements of SAMHSA grants
  • Review charts for treatment compliance and follow up on missed injections
  • Interface with the multi-disciplinary team and participate in development of the interdisciplinary treatment plan
  • Monitor laboratory results and report abnormal findings to physicians
  • Provide emergency care as needed, according to Accordia Health Policy and Procedures
  • Demonstrate ability to recognize crisis states and deescalate or resolve situations
  • Participate in general and medical staff meetings as required
  • Key data into computer to maintain office and patient records
  • Educate community and patients on health issues to increase health literacy
  • May conduct infection control for staff and consumers when indicated
  • Perform related duties as assigned
Requirements
  • Current LPN or RN license in Alabama
  • Basic Life Support (BLS) certification
  • Knowledge in adult/geriatric and/or child/adolescent specialty, to be attained within six months of employment
  • Knowledge of general and psychiatric nursing care, basic knowledge of psychiatric disorders, and safe medication administration practices
  • Demonstration of knowledge of growth and development over the life span, medical terminology, and medications administration
  • Knowledge of behavior management, crisis intervention, and interventions common to acute psychotic and non-violent crisis intervention practice
  • Basic knowledge of appropriate educational methods and understanding of behavioral, psychological, physical health, and social effects of psychoactive drugs
  • Knowledge of trauma-informed care principles and cultural diversity
