Position Summary: The Lead Care Manager (LCM) plays a pivotal role in overseeing the case management of clients and their families, guiding them through the services and programs offered by the Enhanced Care Management (ECM) initiative. The LCM is dedicated to fostering improved health outcomes and overall wellness for our members. This position requires a proactive individual who thrives in both collaborative and independent settings, demonstrating strong interpersonal skills with colleagues and external partners.

Key Responsibilities:

  • Case Management: Manage a designated caseload of adult and pediatric clients, conducting thorough assessments to identify their physical, emotional, and social requirements.
  • Care Planning: Create personalized care plans based on assessment results, taking into account medical history, preferences, and specific needs.
  • Coordination: Facilitate effective communication among healthcare providers, social workers, therapists, and other care team members to ensure a cohesive approach to client care.
  • Monitoring: Regularly assess client progress and adjust care plans as necessary, ensuring adherence to prescribed treatments and interventions.
  • Client Experience: Deliver exceptional client service through various support channels, emphasizing a high-touch, person-centered approach.
  • Documentation: Maintain precise and current records of assessments, care plans, and client interactions, ensuring compliance with applicable regulations.
  • Advocacy: Act as a patient advocate, assisting clients in navigating the healthcare system and understanding their treatment options.
  • Education: Provide informative sessions to clients and their families on health-related topics, treatment choices, and self-care techniques.
  • Resource Referral: Connect clients with relevant community resources and support services to meet their needs.
  • Discharge Planning: Assist in coordinating discharge processes for clients transitioning from healthcare facilities to home or other care environments.
  • Training: Participate in onboarding and training new staff members.
  • Additional Duties: Perform other responsibilities as assigned in accordance with departmental policies.

Qualifications:

  • Education & Experience: Associate Degree with a minimum of 2 years in healthcare or care coordination.
  • Language Proficiency: Fluent in English, both written and verbal.
  • Certifications: Medical Assistant Diploma/Certificate from an accredited institution; valid Driver's License and proof of auto insurance; current BLS certification from the American Heart Association upon hire.
  • Skills: Proficient in computer applications, including email and Microsoft Office Suite; experience with Electronic Medical Records; strong communication skills; problem-solving abilities; capability to work independently and complete tasks within established guidelines; bilingual in Spanish is a plus.

Compensation:

  • Pay Range: $21 - $23 per hour.

Work Environment:

  • Hybrid Work Schedule: 30% remote, 70% in-person client visits, with flexibility in scheduling.
  • Eligible for mileage reimbursement for business-related travel.

Physical Demands: This role involves standard office work and travel, including:

  • Regular use of a computer and office equipment.
  • Occasional lifting of office supplies up to 15 pounds.
  • Ability to operate a vehicle for community visits and meetings.
