Position Summary: The Lead Care Manager (LCM) plays a pivotal role in managing cases for members and their families, guiding them through the services and programs available via the Enhanced Care Management (ECM) initiative. Key Responsibilities:

  • Manage a designated caseload of adult and pediatric members, conducting thorough assessments to identify their physical, emotional, and social requirements.
  • Care Planning: Create personalized care plans that reflect assessment insights, taking into account medical histories, preferences, and specific needs.
  • Coordination: Facilitate effective communication among healthcare providers, social workers, therapists, and other care team members to ensure a holistic approach to patient care.
  • Monitoring: Regularly assess member progress and adjust care plans as necessary, ensuring adherence to prescribed treatments and promptly communicating any significant changes to relevant healthcare providers.
  • Member Experience: Deliver exceptional service to members through various support channels, ensuring a high-touch, person-centered approach.
  • Documentation: Keep precise and current records of assessments, care plans, and member interactions, ensuring compliance with applicable regulations and standards.
  • Advocacy: Act as a patient advocate, assisting them in navigating the healthcare system and understanding their treatment options.
  • Education: Provide valuable information to members and their families regarding health topics, treatment choices, and self-care techniques.
  • Resource Referral: Connect members with suitable community resources and support services to meet their needs.
  • Discharge Planning: Aid in planning and coordinating the discharge process for members transitioning from hospitals or long-term care facilities.
  • Participate in training new staff members as needed.
  • Perform additional duties as required by departmental policies.

Qualifications:

  • Education & Experience: Associate Degree with a minimum of 2 years in healthcare or care coordination.
  • Language: Proficient in English (both written and verbal).
  • Certifications: Medical Assistant Diploma/Certificate from an accredited institution, valid Driver's License, proof of auto insurance, and current BLS certification from the American Heart Association.
  • Skills: Proficient in computer applications, including email, virtual platforms, and Microsoft Office Suite. Experience with Electronic Medical Records is preferred. Strong communication skills, problem-solving abilities, and the capacity to work independently are essential. Bilingual in Spanish is a plus.

Salary Range: $21 - $23 per hour.

Travel Requirements: This role involves a hybrid work schedule, with 30% of duties performed remotely and 70% requiring travel for in-person member visits. Mileage reimbursement is available for business-related travel.

Physical Demands: The position requires standard office work and driving, including operating a computer and other office equipment, as well as traveling to meet with members.
