Salary

The estimated salary for this role is between $55,000 - $65,000 per year, based on experience and qualifications.

Job Description

This position is responsible for providing care coordination for consumers of Gulf Coast Region Assisted Outpatient Treatment Team (GCR AOT). The successful candidate will work closely with courts, community partners, and other entities to support community-based treatment for adults with Serious Mental Illness (SMI) who meet criteria for AOT.

Key Responsibilities:

  • Provide Care Coordination for GCR AOT consumers
  • Assist in implementing a multi-disciplinary AOT program
  • Collaborate with core partners to identify and enroll participants
  • Develop treatment plans and coordinate admission to programs
  • Interface with hospital and court systems
  • Conduct activities to ensure timely access to needed services and supports

Required Skills and Qualifications

To be successful in this role, you will need:

  • A baccalaureate degree or equivalent in the behavioral sciences field of study
  • QMHP certification
  • General knowledge of mental disorders
  • Excellent verbal and written communication skills
  • Ability to work within a team approach
