Job Overview

We are seeking a highly skilled Mental Health Recovery Specialist to join our team at Team1Medical. In this role, you will play a crucial part in ensuring participants receive tailored services to meet their unique needs.

Job Summary

The Patient Care Coordinator is responsible for providing ongoing case management services for applicable participants. This includes monitoring the case through approved closing and maintaining client records and all other required documentation with discretion.

Key Responsibilities
  • Provide individualized support to clients to enhance their recovery journey.
  • Manage crises by assessing, deciding, and acting according to established standards and laws in emergencies involving the health and safety of clients.
Requirements
  • A high school diploma or equivalent is required.
  • At least one year of experience in advocacy, mentoring, education, counseling, substance use service, or case management is necessary.
  • Lived experience with mental illness, addiction, or poverty is preferred.
  • A current valid certification issued and recognized by the Texas Certification Board as a Peer Recovery Support Specialist or equivalent is required.
