The Patient Assessment Coordinator will be responsible for facilitating the admissions process by ensuring the seamless transmission of clinical and benefits information from referral sources to the admissions teams, administration, and third-party payers.
Ideal candidates will possess an RN or LPN license, a degree in Social Work, or other relevant medical credentials.
Estimated Salary Range:
$60,000 - $70,000 annually
Key Responsibilities:
- Conducting comprehensive clinical assessments
- Serving as a resource for liaisons
The Patient Assessment Coordinator achieves these objectives by delivering exceptional customer service and providing accurate, timely, and thorough clinical assessments. This role requires proactive collaboration with all departments within the facility.
Qualified candidates should hold an RN or LPN license in the State of Ohio, a Social Work degree, or other medical credentials. A minimum of three years of treatment experience in a Long Term Care setting is preferred. Previous clinical screening experience and recent healthcare experience in a Long Term Care facility are also advantageous. Candidates should have knowledge of the local medical community and possess an outgoing, energetic personality.
Job Requirements:
- Current, valid, and active license to practice as a Registered Nurse or Licensed Practical Nurse in the state of Ohio, OR a Social Work Degree, OR other medical credentials.
- Valid driver's license required.
- Preferred: Three (3) years of treatment experience in a Long Term Care setting.
- Preferred: Previous clinical screening experience.
- Preferred: Recent healthcare experience in Long Term Care.
- Preferred: Knowledge of the local medical community.
Successful candidates will demonstrate the ability to collect, organize, evaluate, and communicate clinical information professionally and assertively, both verbally and in writing, to medical and administrative staff, patients, and families. Strong customer service skills and a commitment to service excellence are essential.

- Strong organizational, prioritizing, and analytical skills
- The ability to make independent decisions when necessary
