Job Summary

Monument Health is seeking a skilled and compassionate Mental Health Therapist or Social Worker MSW to join our team. As a valued member of our Behavioral Health department, you will play a critical role in providing high-quality patient care and support.

Key Responsibilities:
  • Provide individual, marital, family, and group therapy sessions to patients, developing and implementing personalized treatment plans.
  • Collaborate with multidisciplinary teams, including physicians, to formulate effective treatment plans that address patient needs.
  • Assess patient progress and modify treatment plans as needed, ensuring optimal outcomes.
  • Develop and maintain positive relationships with community providers and hospital staff, facilitating seamless care transitions.
  • Function as a resource and communication coordinator for departmental staff, promoting a culture of teamwork and collaboration.
  • Assist patients in maintaining their sense of competence, identity, and autonomy.
  • Participate in discharge planning, providing direction to discharge planners as needed.
Requirements:
  • Masters degree in Counseling or Social Work.
  • Licensed Professional Counselor (LPC) or Certified Social Worker (CSW) certification.
  • Ability to work in a fast-paced environment, providing high-quality care to patients and families.
