Job Title: Clinical Specialist

We are seeking a highly skilled and compassionate Clinical Specialist to join our team at West River Mental Health. As a Clinical Specialist, you will play a vital role in coordinating treatment, recovery, and support services for individuals with severe and persistent mental illness.

Job Summary:

The Clinical Specialist will work closely with our CARE and IMPACT programs to provide comprehensive services to clients. This includes participating in ongoing assessments and therapeutic engagement, developing and implementing person-centered individualized treatment plans, and coordinating and maintaining up-to-date assessments and evaluations.

Responsibilities:
  1. Participate in the ongoing assessment and therapeutic engagement of clients
  2. Work with consumers and staff in developing and implementing person-centered individualized treatment plans
  3. Coordinate and maintain up-to-date assessments and evaluations to establish eligibility for services
  4. Participate in treatment planning and monitoring progress towards goals and objectives
  5. Assist in locating, coordinating, and monitoring medical, social, co-occurring, and psychiatric services
  6. Assist in planning and coordinating clients' financial resources, including vocational opportunities and training
  7. Ongoing assessment of the client's symptoms and response to treatment/education
  8. Provide direct services or referral, including social and emotional support, coupled with collaborative support to team clients
  9. Coordinate the teaching of daily living skills, including grocery shopping, hygiene, maintaining clean housing, utilizing public transportation, maintaining self-care, exploring employment opportunities, and instructing clients in the act of self-administration of prescription and nonprescription medication
  10. Transport and/or accompany clients to appointments that connect them to community resources and services
  11. Act as an advocate for clients to secure needed services and financial entitlements
  12. Locate and/or establish self-help and support groups for team clients, families, and/or significant others
  13. Attend all team meetings to facilitate an integrated, multidisciplinary team approach to service
  14. Maintain compliance with all applicable BMS practice standards, guidelines, and HIPAA requirements
  15. Complete all required documentation in a timely manner consistent with agency guidelines
  16. Maintain department-required productivity and standards
  17. With documented consent, engage the client's family and/or significant others to provide education regarding mental illness and system resources
  18. Encompass WRMH mission, philosophy, code of ethics, vision, principles, and chosen values to support a positive working environment and continuous quality improvement among staff
Requirements:

To be successful in this role, you will need:

  1. A minimum of a High School Diploma or equivalent, with a preference for working with people in a case management role
  2. Associate's/Bachelor's degree in a human service field and experience preferred
  3. Ability to assess clients by phone and face-to-face in emergency situations
  4. Ability to interact professionally with clients and customers
  5. Participate as an active team member, working cooperatively with others, providing coaching or mentoring, and exhibiting a positive attitude
  6. Ability to work evening or weekend hours as required on an as-needed basis
  7. Ability to maintain a South Dakota driver's license, vehicle liability insurance, and safely operate a vehicle in all weather conditions
