Job Description

Inova Health System is seeking a highly skilled Registered Nurse (RN) to fill the role of Oncology Nurse Navigator. This position requires a strong clinical background, excellent communication skills, and the ability to work effectively with patients, families, and healthcare teams.

Key Responsibilities
  • Guide patients and families through the healthcare system, ensuring seamless care coordination and navigation.
  • Develop and implement individualized care plans, taking into account patients' unique needs, preferences, and circumstances.
  • Collaborate with multidisciplinary teams to address patient questions and concerns, and provide education on treatment options, community resources, and self-care management.
  • Act as a liaison and advocate for patients, facilitating access to necessary healthcare services and resources.
  • Conduct comprehensive assessments of patients' health and psychosocial needs, and develop care plans in collaboration with healthcare providers and patients.
  • Participate in quality improvement efforts, promoting evidence-based practice and lifelong learning.
  • Build effective working relationships with healthcare providers, community resources, and patients to maximize patient outcomes and identify/resolve barriers to care.
Requirements
  • 3-5 years' progressive clinical nursing experience with primary care management of patient populations.
  • BSN degree.
  • Current Basic Life Support (BLS) certification.
  • Certification in oncology within 2 years of employment.
  • Strong oncology knowledge, basic knowledge of managed care, working knowledge of insurance coverage and procedures, working knowledge of community resources, understanding of healthcare facilities and processes, and proficient computer skills.
