Lead Instructor for Unit Success

We are seeking a highly skilled Habilitation Specialist to lead our unit and contribute to the growth and development of our participants. This role involves designing and implementing individual program plans, daily activity schedules, and community integration initiatives.

Key Responsibilities:
  • Ensure all unit activities enhance and support participant social responsibility, independence, dignity, and continued growth.
  • Participate in Interdisciplinary Team Conferences and act as liaison between programs to ensure seamless communication.
  • Complete documentation and paperwork within specified timeframes, including Annual and Semi-Annual day habilitation and staff action plans.
  • Plan, write, and implement goals designed to maximize participant independence, as determined by the IDT.
  • Monitor progress by collecting, documenting, and analyzing performance data.
  • Establish and implement activity schedules for each individual, ensuring a stimulating and safe environment.
  • Communicate with residences and/or families regarding progress, incidents, etc., while maintaining confidentiality at all times.
Requirements:
  • Associates degree in Education, Human Services, or related field.
  • Valid NYS driver's license and two years' experience working with people with developmental disabilities.
Salary Information:

The estimated salary for this position is $18.68 per hour, based on the location and job requirements.
