Summary:
The LPN is tasked with overseeing and coordinating medication management and resident care delivery.

Moreover, the LPN assists the Director of Nursing in establishing nursing care standards and managing residents under a physician's guidance.


Key Responsibilities:


Facilitates and accurately aids residents in the self-administration of prescribed medications in accordance with individual medication care records.

Organizes medication carts to enhance accuracy and efficiency during medication distribution.
Maintains an inventory of necessary medications, monitors expiration dates, and ensures proper storage. Adheres to controlled substance protocols and guarantees the appropriate disposal of expired or excess medications.

Evaluates medication administration and alerts the Director of Nursing of any potential issues with dispensing instructions or adverse drug interactions.

Ensures that all resident medication records are precise, complete, and that all required forms are filed.

Assists in the examination and treatment of residents, executing orders as directed by the Director of Nursing or their designee.

Informs the Director of Nursing and relevant neighborhood directors of any changes in a resident's physical or mental condition.

Supports the Director of Nursing with scheduling and following up on physician appointments, ensuring medication orders are accurately documented and executed.

Maintains comprehensive knowledge of emergency protocols and responds to all emergency situations. Performs first aid and/or CPR as necessary.
Ensures all resident records are accurate. Completes and signs documentation, resident care notes, medication administration records, and incident reports, providing follow-up as needed. Upholds confidentiality of resident and community information.

Conducts interviews and assessments of current residents to evaluate needs, monitors and records changes in resident medical conditions and behaviors, assists in the formal reassessment of resident care needs, and recommends adjustments to care plans.

Participates in the planning for resident move-ins, transfers, and discharges.
Collaborates effectively with residents, their families, community staff, physicians, consultants, and ancillary service providers. Acts as a liaison between the community and residents' supervising physicians.
Guides neighborhood team members and residents in executing emergency and disaster protocols during drills and actual events.
Provides daily updates to the Director of Nursing regarding community conditions, resident status, and nursing supply needs.
Engages in professional development activities as required.
Reports any suspicions or evidence of elder abuse, mistreatment, or neglect as outlined in Personnel Policies.

Knowledge and Skills:
Excellent verbal communication skills and interpersonal abilities are essential.
Ability to work effectively in a team-oriented environment.
Strong organizational and time management skills.
Proficiency in calculating figures related to budgetary items, staffing needs, medication dosages, and percentages.
Familiarity with applicable health care and employment regulations at both Federal and State levels.
Proficient in computer applications, including Word, Excel, Outlook, PowerPoint, and Property Management Systems.

Qualifications:


Must communicate clearly and effectively with individuals of all ages and abilities, demonstrating respect for their rights and sensitivities while maintaining a positive demeanor.

Proficiency in reading and writing English, and following verbal and written instructions is required.
Must present a clean and professional appearance and adhere to the dress code outlined in the employee handbook.
Flexibility to work varied hours, including weekends and evenings.
Must be in good health, physically and mentally capable of performing assigned duties.
Demonstrates freedom from pulmonary tuberculosis through a health screening conducted by a physician within the specified timeframe.
Must pass a criminal background check as required.
A minimum of three years of experience in a similar role, preferably in retirement living or home care.
Ability to read, analyze, and interpret human service literature, professional journals, technical procedures, and governmental regulations.
Ability to write reports, business correspondence, and procedural manuals.
Ability to effectively present information and respond to inquiries from managers, clients, customers, and the public.
A high school diploma is required; a college degree is preferred.
Must hold a valid LPN/LVN license.

Mental and Physical Requirements:
While performing the duties of this position, the team member is regularly required to communicate verbally and audibly. Specific vision abilities required include close vision and the ability to adjust focus.

The team member is frequently required to stand, walk, sit, use hands to handle or feel objects, tools, or controls; and reach with hands and arms above, below, and at shoulder height.

The team member regularly lifts and/or moves up to 25 pounds.

Must be capable of pushing a minimum of 200 pounds a distance of at least 200 feet.

Assistance in the evacuation of residents during emergencies may be necessary.
Work environments may be subject to noise, distractions, and interruptions. The ability to remain calm and positive in stressful situations is essential.
The COVID-19 vaccination is a condition of employment.
