The Hospice Admissions Registered Nurse (RN) will adhere to public health nursing standards, providing and coordinating hospice services for patients and their families in collaboration with an interdisciplinary team.

The Admissions RN is responsible for conducting initial nursing evaluations, making informed clinical decisions based on the unique needs and preferences of patients and families, utilizing professional expertise.

This position involves overseeing LPNs, CNAs, Unit Secretaries, and Volunteers, ensuring high-quality care delivery.

Qualifications:

  • A minimum of two years of experience in public health, community health, medical-surgical, or related nursing fields. Experience in home care, hospice, or oncology nursing is preferred.
  • A Diploma or Associate's degree in Nursing from an accredited institution is required.
  • Current Healthcare Provider BLS (CPR) certification is mandatory for practice.

Key Responsibilities:

  • Facilitate appropriate referrals to other healthcare disciplines and community resources.
  • Implement preventive and rehabilitative nursing interventions to enhance patient quality of life.
  • Collaborate with the regional and admissions teams, demonstrating flexibility in scheduling and shared responsibilities for patient care.
  • In partnership with the Clinical Supervisor and Executive Director, lead orientation, training, and mentorship for new hospice staff and volunteers.
  • Engage in quality improvement initiatives and ongoing education to enhance professional skills and knowledge of current nursing practices.

Compliance and Care Coordination:

  • Communicate the hospice philosophy, objectives, and services to patients and families to ensure comprehensive understanding.
  • Work with the Team Social Worker to facilitate patient admissions.
  • Determine the most suitable hospice program to address patient and family needs.
  • Develop and initiate patient care plans in accordance with approved treatment protocols, collaborating with patients, families, and the interdisciplinary team.
  • Document all nursing care and communications in the medical record promptly and accurately, adhering to organizational policies.
  • Educate and supervise family members or caregivers in providing necessary support for the patient’s nursing and personal care needs.
  • Prepare and manage clinical documentation related to nursing services delivered.
  • Deliver or oversee skilled nursing services as outlined in the care plan, focusing on palliative, preventive, and rehabilitative care in accordance with hospice policies.

This role requires consent to drug and/or alcohol testing following a conditional employment offer, along with ongoing adherence to the Drug-Free Workplace Policy.
