Job Overview

Position Title: Information and Referral Specialist

Compensation:

EXEMPT: $46,300 – $49,400 annually

Work Environment:

This role is fully remote. The standard operating hours are Monday to Friday, 8:30 AM to 4:30 PM (CST). The organization primarily supports remote work, with some roles requiring a hybrid schedule.

Expected Work Hours:

  • For Eastern or Central Time Zones: 12:00 PM to 8:00 PM
  • For Mountain or Pacific Time Zones: 10:00 AM to 6:00 PM
  • Flexibility to work evenings and one Saturday per month is required.

Note: This is a grant-funded position.

Language Requirement: Bilingual in English and Spanish is essential.

Position Summary:

This role involves conducting outreach to farmworkers and managing a national information and referral hotline, aiding farmworker families in accessing healthcare and preventive services.

Qualifications:

Required:

  • Technical knowledge in community outreach and patient navigation with 1-2 years of higher education or certification as a community health worker.
  • Experience in outreach, patient navigation, and familiarity with immigrant communities and resources.
  • Strong verbal communication skills.
  • Bilingual in English and Spanish.
  • Experience with digital communication platforms.

Preferred:

  • Community Health Worker certification.

Key Responsibilities:

  • Call for Health Program:
    • Respond to inquiries, providing information or referrals to healthcare services.
    • Offer information on COVID-19 testing and vaccination, including referrals and follow-ups.
    • Assist with interpretation between healthcare providers and patients.
    • Assess callers' social and economic needs, providing necessary referrals.
    • Screen applications for financial assistance and coordinate services.
    • Document interactions and update information in the database.
    • Research new healthcare resources.
    • Promote awareness of the program and participate in fundraising efforts.
  • USDA Grant Activities:
    • Conduct outreach to verify agricultural employment and collect necessary data.
    • Assess urgency of needs to expedite assistance.
    • Arrange interpretation services as needed.
    • Provide information and education about healthcare services.
    • Follow up on pending cases and document encounters.

Work Environment:

Work is primarily technical and administrative, requiring knowledge of organizational policies. Duties are generally performed independently, with oversight from the manager.

Physical Requirements:

Regularly required to work at a designated workspace for extended periods. Must be able to use a computer and office tools efficiently.

Leadership:

Receives general supervision and does not supervise other staff.

Must be a U.S. citizen or authorized to work in the U.S. Must be at least 18 years old.
