**About the Role

We are seeking a highly motivated and compassionate Community Support Specialist to join our team at RHA Health Services. As a key member of our START Program, you will play a vital role in supporting individuals with developmental disabilities and mental health challenges in their communities.

Key Responsibilities:

  • Develop and implement individualized support plans to promote community integration and independence.
  • Provide crisis intervention and management services as needed.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination.
  • Maintain accurate records and documentation of client progress.

Requirements and Qualifications:

  • Bachelor's degree in Social Work, Human Services, or related field required; Master's degree preferred.
  • Minimum 2 years of experience working with individuals with developmental disabilities and mental health needs.
  • Valid driver's license and reliable vehicle for travel required.
  • CPR, First Aid, and NCI certifications a plus.
