Transplant Coordinator Specialist

Mount Sinai Health System is seeking a highly skilled Transplant Coordinator to join our team. As a key member of our Transplant Institute, you will play a critical role in the coordination of clinical programs and services for designated patient populations.
Responsibilities:- Develop, implement, and evaluate clinical programs and services for designated patient populations.
- Coordinate the multidisciplinary screening and evaluation of patients seen in the transplant practice at MSH and approved satellites.
- Ensure all necessary lab tests are performed on potential transplant candidates and that results are updated as required.
- Participate in making recommendations regarding placing patients on the transplant waiting list.
- Communicate with all active transplant candidates regarding waiting list status on an ongoing basis.
- On call responsibilities include facilitating all organ offers from UNOS and communicating with surgeons, fellows, and patients to ensure timeliness of all organ offers.
- Follow patients during their hospitalization and coordinate with the transplant team to coordinate consulting services as needed.
- Coordinate outpatient follow-up visits with the outpatient transplant team.
- Assess continued recovery of patients through compliance with outpatient visits, scheduling of blood tests, regular review of labs, and adjusting immunosuppressive medication as needed during regular lab review.
- Baccalaureate Degree in Nursing is required.
- Masters in Nursing and certification in specialty area are preferred.
- Progressive job-related experience specific to the area of responsibility.
- New York State Nurse/NP/PA License with current registration.
