Transplant Coordinator

We are seeking a highly skilled and compassionate Transplant Coordinator to join our team at Mount Sinai Hospital. As a key member of our Transplantation Institute, you will play a vital role in coordinating clinical programs and services for designated patient populations.
Responsibilities:- Develop, implement, and evaluate clinical programs and services for designated patient populations.
- Coordinate the multidisciplinary screening and evaluation of patients seen in the transplant practice at MSH and approved satellites.
- Ensure all necessary lab tests are performed on potential transplant candidates and that results are updated as required.
- Communicate with all active transplant candidates regarding waiting list status on an ongoing basis.
- On call responsibilities include facilitating all organ offers from UNOS and communicating with surgeons, fellows, and patients to ensure timeliness of all organ offers.
- Follows patients during their hospitalization and coordinates with transplant team to coordinate consulting services as needed.
- Educates patients and family regarding all transplant medications, lifestyle changes, as well as outpatient follow-up protocols.
- NY RN with Baccalaureate Degree in Nursing is required.
- Masters in Nursing and certification in specialty area are preferred.
- At least 2 years acute care nursing, critical care nursing preferred.
