RN Transplant Coordinator-Mt Sinai Hospital Liver Transplant Services

The Transplant Coordinator is a key member of the transplant team, responsible for coordinating the care of patients undergoing liver transplantation. This role requires strong clinical knowledge, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Responsibilities- Coordinate the multidisciplinary care of patients undergoing liver transplantation, including pre-transplant evaluation, post-transplant care, and follow-up appointments.
- Collaborate with transplant surgeons, hepatologists, nephrologists, social workers, nutritionists, pharmacists, and finance staff to ensure seamless care coordination.
- Develop and implement clinical programs and services for designated patient populations, including education and support for patients and families.
- Participate in the development and implementation of policies, procedures, and standards of care for designated patient populations.
- Contribute to the recruitment and retention of support staff, including transplant coordinator assistants and medical assistants.
- NYS RN with BSN and BCLS, and at least 2 years of acute care nursing experience, critical care experience, and/or specific experience with patient population preferred.
- Proven progressive responsibilities and ability to work effectively in a fast-paced environment.
