Job Description

The Transplant Coordinator is a key member of the Transplantation Institute team, responsible for coordinating clinical programs and services for designated patient populations. This role requires a Registered Professional Nurse, Nurse Practitioner, or Physician Assistant with specific knowledge and skills.

Responsibilities
  • Develop, implement, coordinate, and evaluate clinical programs and services for designated patient populations.
  • Coordinate the multidisciplinary screening and evaluation of patients seen in the transplant practice at Mount Sinai and approved satellites.
  • Ensure all necessary lab tests are performed on potential transplant candidates and that results are updated as required.
  • Participate in making recommendations regarding placing patients on the transplant waiting list.
  • Communicate with all active transplant candidates regarding waiting list status on an ongoing basis.
  • Follow patients during their hospitalization and coordinate with the transplant team to coordinate consulting services as needed.
  • Educate patients and family regarding all transplant medications, lifestyle changes, and outpatient follow-up protocols.
Requirements
  • NYS RN with BSN and BCLS, and at least 2 years of acute care nursing experience, critical care experience, and/or specific experience with patient population preferred.
  • Proven progressive responsibilities are essential.
