In this role, you will deliver crucial case management and clinical coordination services in alignment with the Arizona Practice Model's 12 Principles. Work collaboratively with colleagues to assist families in navigating available resources both within Touchstone and the broader community.
Qualifications:
- Minimum age of 21 years
- Bachelor's degree or relevant experience in behavioral health services
- Valid Arizona Driver's License and Fingerprint Clearance Card
Key Responsibilities:
1. Engage with families to identify necessary supports and facilitate Child and Family Team Meetings.
2. Coordinate service plans and meetings in accordance with established guidelines.
3. Educate families about the behavioral health process and assist them in accessing available resources.
4. Conduct screenings and referrals while maintaining electronic documentation.
5. Build and nurture relationships with consumers, families, and community stakeholders.
6. Perform additional duties as assigned.
