Key Responsibilities

As a Community Health Advocate, you will:

  • Support members in managing their health through education, peer support, and resource navigation
  • Assist in motivating behavioral changes to improve health outcomes
  • Coordinate medical and social services appointments, ensuring seamless transitions in care
  • Maintain accurate documentation of patient interactions and progress
Requirements and Qualifications

To be successful in this role, you must possess:

  • A G.E.D./High School diploma or equivalent, plus 3+ years of relevant experience
  • Bachelor's degree preferred, with 1-2 years of experience in a related field
  • Demonstrated expertise in working with vulnerable populations, including those experiencing homelessness and mental health/substance use conditions
  • Excellent oral and written communication skills
  • Ability to work independently and as part of a team
