Job Description:Role SummaryThe Community Services Liaison is responsible for providing support and services to elderly and physically challenged residents living in TRM housing communities. This includes assessing resident needs, developing individualized care plans, and coordinating with healthcare providers and social services agencies to ensure that residents receive the care and support they need.
- Provide comprehensive case management services to residents, including assessments, goal setting, and progress monitoring.
- Develop and implement individualized care plans that address resident needs and goals.
- Coordinate with healthcare providers, social services agencies, and other stakeholders to ensure that residents receive necessary care and support.
- Communicate regularly with residents, family members, and caregivers to ensure that needs are met and concerns are addressed.
The Community Services Liaison will be responsible for the following duties:
- Serving as a resident advocate and providing support and guidance to residents and their families.
- Assessing resident needs and developing individualized care plans.
- Coordinating with healthcare providers and social services agencies to ensure that residents receive necessary care and support.
- Providing education and resources to residents on topics such as aging in place, health and wellness, and community resources.
- Collaborating with internal teams, including property management and maintenance, to ensure that residents' needs are met.
- Maintaining accurate and up-to-date records of resident interactions, care plans, and outcomes.
To be successful in this role, the Community Services Liaison must possess the following qualifications:
- Bachelor's degree in Social Work, Gerontology, Psychology, or a related field.
- Minimum 1 year of experience in a social work or healthcare setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
- Knowledge of community resources and services available to seniors and individuals with disabilities.
The salary range for this position is $55,000 - $65,000 per year, depending on experience. We offer a competitive benefits package, including medical, dental, and vision insurance, 401(k) retirement plan, and paid time off.
