Job Overview

The Heritage Health Network is a pioneering healthcare organization dedicated to enhancing the health and wellness of vulnerable populations in Riverside and San Bernardino counties.

We are seeking a skilled Community Outreach Coordinator to drive member enrollment and expand our reach within the community. This position focuses on identifying and engaging potential members, building strategic relationships with healthcare providers, and representing HHN at community events and meetings.

Key Responsibilities:
  • Enrollment Growth: Develop and execute targeted outreach strategies to identify, engage, and enroll potential HHN members, particularly in Riverside and San Bernardino counties.
  • Partnership Development: Establish and maintain strong relationships with healthcare providers, Community-Based Organizations (CBOs), foster agencies, and county facilities to increase referral sources and membership opportunities.
  • Community Representation: Serve as the face of HHN in community events, health fairs, and meetings, advocating for the health needs of underserved populations and promoting HHN's services.
  • Sales-Focused Outreach: Your goal is to enroll members into our program. You will visit doctor's offices and county facilities to educate them about HHN and the benefits their patients and clients will receive in our program.
  • Event Coordination: Organize and lead community health events and informational sessions to increase awareness of HHN's services and ECM program.
  • Feedback Collection: Gather and analyze feedback from community members, stakeholders, and partners to refine outreach strategies and improve enrollment processes.
  • Collaborative Efforts: Work closely with HHN's marketing and care management teams to align outreach and engagement strategies with overall business goals.
Required Skills & Qualifications:
  • Bachelor's degree in Business, Sales or Marketing, or a related field (equivalent experience will be considered).
  • Proven experience in community outreach, sales, member recruitment, or public health advocacy, preferably within healthcare settings.
  • Strong understanding of cultural dynamics and health needs in Riverside and San Bernardino counties.
  • Excellent sales, communication, and interpersonal skills, with the ability to engage and influence diverse groups effectively.
  • Knowledge of local community health resources and networks.
  • Ability to work independently and as part of a team to achieve enrollment goals.
  • Bilingual skills are highly desirable.
Estimated Salary:

$65,000 - $85,000 per year, depending on experience.
