Heritage Health Network is a pioneering healthcare organization dedicated to enhancing the health and wellness of vulnerable populations in Riverside and San Bernardino counties.

We are seeking an experienced Enrollment and Outreach Coordinator to join our team. As a key member, you will drive member enrollment and expand our reach within the community.

Key Responsibilities
  • Develop targeted outreach strategies to identify and engage potential HHN members, particularly in Riverside and San Bernardino counties.
  • Establish and maintain strong relationships with healthcare providers, Community-Based Organizations (CBOs), foster agencies, and county facilities to increase referral sources and membership opportunities.
  • Serve as the face of HHN in community events, health fairs, and meetings, advocating for the health needs of underserved populations and promoting HHN's services.
  • Visit doctor's offices and county facilities to educate them about HHN and the benefits their patients and clients will receive in our program.
  • Organize and lead community health events and informational sessions to increase awareness of HHN's services and ECM program.
  • Gather and analyze feedback from community members, stakeholders, and partners to refine outreach strategies and improve enrollment processes.
Requirements
  • Bachelor's degree in Business, Sales, or Marketing, or a related field (equivalent experience will be considered).
  • Proven experience in community outreach, sales, member recruitment, or public health advocacy, preferably within healthcare settings.
  • Strong understanding of cultural dynamics and health needs in Riverside and San Bernardino counties.
  • Excellent sales, communication, and interpersonal skills, with the ability to engage and influence diverse groups effectively.
  • Knowledge of local community health resources and networks.
  • Ability to work independently and as part of a team to achieve enrollment goals.

The ideal candidate will have a proven track record of achieving or exceeding enrollment targets and a familiarity with the Healthcare system.

We offer a competitive salary of $55,000 - $65,000 per year, depending on experience, plus commission and benefits including health insurance, sick time, paid holidays, work flexibility, and annual merit increases.
