Role Responsibilities:
The Family Support Case Coordinator will engage with 10-20 families within our Shelter, Prevention, and Rapid Rehousing Programs. This role encompasses both community outreach and on-site support at the Patrick Henry Family Shelter. Utilizing a housing-first model complemented by a trauma-informed care approach, the primary objective is to assist families in securing and sustaining housing. Key responsibilities include:
- Conducting individualized needs assessments and developing tailored service plans.
- Identifying and pursuing new housing opportunities and resources in the local area.
- Addressing safety concerns with empathy while guiding families back to their housing objectives.
- Coordinating Family Shelter Planning Team meetings to facilitate housing strategies.
- Collaborating with families to outline necessary supports for overcoming housing barriers.
- Linking clients to essential services, including employment and financial assistance.
- Providing education on financial management and tenant responsibilities.
- Building relationships with landlords to assist families facing multiple challenges.
- Documenting all client interactions and maintaining comprehensive client records.
- Participating in multidisciplinary team efforts to ensure timely service delivery.
- Committing to continuous learning and improvement based on best practices.
Candidate Profile:
Required Qualifications:
- Bachelor's degree in human services or a related field, or equivalent experience.
- Fluency in Arabic or Spanish, in addition to English.
- Minimum of 1 year of experience in case management or direct service roles.
- Strong communication skills, both written and verbal.
- Ability to manage competing priorities effectively.
- Proficient in Microsoft Office Suite applications.
- Valid Driver's License and reliable transportation.
Preferred Qualifications:
- Experience working with clients facing homelessness, mental health, or substance abuse challenges.
- Familiarity with the Homeless Management Information System (HMIS).
