Job Overview

PRIMARY FUNCTION:

Supports the Director of Social Work in managing the admissions, transfers, and discharges of residents at Menorah Park. Provides essential direct and indirect support to residents and their families, addressing emotional, social, economic, and physical challenges while fostering effective communication with staff, residents, and community resources.

KEY RESPONSIBILITIES:

  1. Facilitates the admissions, transfers, and discharges of residents within Menorah Park.
  2. Guides residents through initial adjustments, interpersonal dynamics, policy navigation, financial matters, and unique needs.
  3. Assists residents in coping with emotional challenges related to health issues, disabilities, and family dynamics.
  4. Maintains regular communication with family members regarding resident concerns and updates.
  5. Keeps accurate case records and prepares statistical reports as necessary.
  6. Participates in relevant staff meetings and patient care discussions.
  7. Represents Menorah Park in community engagements and seminars as directed by the Executive Director.
  8. Stays informed about advancements in social work practices, particularly in gerontology, and applies new techniques.
  9. Completes all mandatory training and in-service education sessions.
  10. Assists with administrative tasks as needed.
  11. Performs additional duties as assigned by the Director of Social Work.

QUALIFICATIONS:

A High School Diploma or GED is preferred. The role requires maturity, patience, and empathy to effectively support elderly and disabled individuals and their families in various circumstances.
