Job Summary

We are seeking a highly skilled and compassionate Social Worker MSW to join our team at Loretto Management Corporation. As a key member of our interdisciplinary care team, you will provide comprehensive social work services to PACE participants, including psychosocial evaluation and intervention, case management, and counseling.

Responsibilities
  • Presents social work concerns and issues to the interdisciplinary team, suggesting interventions and recommendations for individual participants.
  • Confers with participants and families to discuss individual expectations, long-term goals, and short-term goals, and attends family meetings as needed.
  • Attends morning meetings to review significant events and occurrences related to family, finance, and psychosocial/behavioral changes, making recommendations for interventions and care plan changes.
  • Intervenes in crisis situations, advocating for participants' needs, wishes, and rights to self-determination.
  • Participates in the development and revision of participants' plans of care as a member of the interdisciplinary team, integrating social work treatment plans into the overall plan of care.
  • Assesses participants, families, and home environments for areas where social work planning and interventions can be of assistance, maintaining required documentation in the EMR.
  • Assists in determining participants' and families' needs related to social supports, psychological supports, financial supports, and counseling, as well as housing options and alternatives.
  • Collaborates with participants and families in making necessary applications for public assistance, housing placement, pre-admission screening, and other services.
  • Performs pre-enrollment assessments of potential enrollees, develops treatment plans in cooperation with participants, families, and team members, and performs semiannual and annual assessments.
  • Maintains active communication with other team members and nursing homes to discuss progress and coordination of client care efforts.
  • Assists in the development and implementation of social work policies and procedures, collaborating with the QAPI Coordinator on the QAPI program and serving on task forces as requested.
  • Educates other team members on community resources, Medicaid, and other social programs and psychosocial issues related to client care, maintaining current knowledge of community resources and new applicable regulations.
  • Counsels and instructs paraprofessional staff in psychosocial and communication issues, assists with difficult family/care-giver conflicts, and supervises BSW and MSW students as appropriate.
Requirements
  • Licensed by the NYS Education Department as a Social Worker.
  • Master of Social Work degree from an accredited program.
  • Minimum of one year professional experience with geriatric care, long-term healthcare, or community services to the aging in a direct service capacity.
  • Excellent written and verbal communication skills.
  • Demonstrated organizational, problem-solving, and conflict resolution skills.
  • Strong interpersonal skills.
  • Proficient in MS Office programs and able to learn new software rapidly.
