Key Responsibilities

The Scheduling Coordinator is tasked with overseeing patient referrals and visit arrangements. This role involves assigning patient assessments and other appointments as directed by healthcare providers through an online scheduling platform. Collaboration with the Team Leader is essential to match clinicians with the appropriate skills to meet patient requirements.
  • Employs electronic medical records (EMR) and scheduling systems to effectively coordinate all visits with field personnel, considering business, patient, and employee needs.
  • Manages workflows for scheduled, missed, rescheduled, reassigned, declined, and completed visits.
  • Regularly reviews Pending Referrals and assigns licensed professionals and patient care managers for all start of care visits.
  • Maintains daily communication with field staff regarding any unresolved visits that are late, pending, or incomplete to ensure timely resolution.
Qualifications

Educational Background
  • High school diploma or equivalent
Experience
  • Preferred: At least one year of scheduling experience in a healthcare environment utilizing an online scheduling system.
Skills
  • Strong organizational, customer service, communication, and decision-making abilities are essential.
  • Familiarity with state and federal regulations governing OASIS visits, supervisory, and reassessment visits is beneficial.
