Per Diem LPN Home Health Nurse

BAYADA Home Health Care is seeking a skilled Licensed Practical Nurse (LPN) to join our team in Cape May Court House, New Jersey. As a per diem LPN, you will provide high-quality home health care to adult and geriatric clients in their own homes.
Responsibilities- Make home visits to clients in designated territories to provide skilled nursing and rehabilitative care.
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
- Monitor clients' conditions and report changes to the Clinical Manager and Client Services Manager.
- Follow up with, execute, and properly document doctors' orders.
- Perform client assessments as necessary.
- Case management and coordination.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
- A current license as a Licensed Practical Nurse in the state of New Jersey.
- A minimum of one year of recent, verifiable experience as an LPN.
- Graduation from an accredited and approved nursing program.
- Prior home care experience is strongly preferred.
- Demonstrated ability to read, write, and effectively communicate in English.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills and prior experience with electronic medical records (EMR) are preferred.
