VNA Health Group is seeking a skilled Registered Nurse to join our team as a Case Manager in Home Health. This is a dynamic opportunity to provide high-quality care to patients in their homes.
Responsibilities- Provide comprehensive nursing care to patients in their homes, including assessment, planning, and evaluation of care.
- Collaborate with healthcare providers, patients, and families to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Document patient information accurately and thoroughly, including progress notes and care plans.
- 1-2 years' experience in Med Surg or hospital setting, preferred.
- Completion of an accredited RN education program with a diploma, Associate's or Bachelor's degree. Bachelor's degree in Nursing preferred.
- Registered Nurse, licensed in the State of Ohio
- Experience in home health care and 1 year experience in medical-surgical nursing. In addition to Acute Care experience, preferred.
- Valid driver's license and CPR certification is required.
