Job Description:

We are seeking an experienced Speech Language Pathologist to join our team at Rehab Without Walls - Home and Community Program. This program specializes in providing functional rehabilitation services to individuals with complex neurological injuries in the comfort of their own homes and communities.

About the Role:

This position offers a unique opportunity for clinicians to work autonomously while being part of a collaborative team. As a Speech Language Pathologist, you will be responsible for assessing patients' communication and swallowing disorders, developing treatment plans, and implementing interventions in various settings such as homes, schools, workplaces, and community environments.

Responsibilities:
  • Evaluate patients' speech, language, cognitive, and swallowing disorders and develop individualized treatment plans.
  • Regularly reassess treatment plans, attend staffing meetings, and family conferences to ensure effective care coordination.
  • Perform discharge evaluations, make recommendations for home and community safety, and provide referral resources.
  • Communicate patients' needs and progress to the treatment team, physicians, caregivers, and family members.
Requirements:
  • A Master's degree from an accredited Speech Language Pathology program.
  • A valid Speech Language Pathology license in the state(s) of practice.
  • Current CPR certification in accordance with state regulations.
  • A minimum of one year's experience as a Speech Language Pathologist.
Estimated Salary Range:

$50-$55 per hour.
