Salary Estimate: $65,000 - $80,000 per year

Job Description

This role involves caring for pediatric patients transitioning from hospital to home for the first time. As a Transitional Care Team Nurse, you will provide nursing care and assist in the transition process, working closely with hospitals and agencies to facilitate discharge and prevent re-hospitalization.

  • Provide skilled nursing care to pediatric patients in their homes
  • Assist with hospital discharge planning and coordination
  • Support patients and families during the transition process
  • Collaborate with interdisciplinary teams to ensure seamless care delivery

Required Skills and Qualifications

To succeed in this role, you will need:

  • A current state license as a Registered Nurse (BSN preferred)
  • A minimum of two years of nursing experience in a Home Health or Acute setting
  • Trach and Vent experience, NICU/PICU experience preferred but not required
  • Current CPR card and Current health certificate as applicable
  • Reliable transportation and automobile insurance
