Role: Rehabilitation Therapy Assistant for an In-house Skilled Nursing Facility
Employment Type: PRN position available
Objective: To support the planning, organization, development, and direction of Rehabilitation Therapy Assistant services in compliance with current applicable federal, state, and local standards, guidelines, and regulations, as directed by the Director of Rehabilitation, Administrator, or Regional support team, ensuring the highest quality of resident care is consistently upheld.
Key Responsibilities:
- Deliver rehabilitation therapy assistant services to residents in alignment with established standards of practice, company protocols, and departmental procedures.
- Comply with company protocols regarding documentation and billing of rehabilitation therapy assistant services.
- Exhibit sound judgment in the assessment, planning, execution, and follow-up of resident therapy programs.
- Perform regular screenings of residents to assess the need for intervention or treatment.
- Conduct timely evaluations of residents referred for rehabilitation therapy to ascertain the necessity for intervention or treatment.
- Gather necessary data for treatment planning and implementation through resident evaluations.
- Carry out specialized assessments as required.
- Formulate treatment plans by setting short-term and long-term objectives and strategies to achieve identified goals.
- Interpret and communicate evaluation results, treatment plans, and recommendations to residents, families, and interdisciplinary team members.
- Complete compliant and medically necessary documentation in accordance with CMS expectations using the NetHealth EMR system.
Qualifications:
- Minimum of an Associate's Degree in Physical Therapy from an accredited institution.
- Possession of an active Physical Therapy license in the relevant state(s).
- Commitment to ongoing continuing education and professional development to maintain current licensure and certification.
