Job Overview

Summary:

The role involves managing and coordinating care for clients and their families within a residential treatment environment. This includes individual and family case management services aimed at enhancing client and family participation in treatment, facilitating educational and skill-building groups in partnership with the clinical team, and overseeing admission, discharge planning, and coordination with external providers.

Work Schedule:

8:00 AM - 4:00 PM

Key Responsibilities:

  • Facilitate client engagement through the organization of appointments, aftercare plans, schedules, and meetings in collaboration with the treatment team.
  • Maintain communication with referral sources and external providers through consistent, professional interactions, assessing needs, and documenting contacts throughout the treatment process. This may include coordination for utilization review, legal matters, and employment support for clients.
  • Deliver case coordination services, including arranging transportation for clients.
  • Conduct individual case coordination sessions.
  • Assist in the formulation of aftercare plans, providing referrals and establishing contact with aftercare providers to address external needs.
  • Support clinical services by facilitating groups and engaging clients weekly to promote program participation.
  • Engage in collaborative treatment team discussions and maintain professional communication with counselors and the treatment team.
  • Document interactions with clients, families, referrals, and external providers in Carelogic, adhering to program timelines.
  • Collaborate with the client navigation team to address aftercare requirements.
  • Provide on-site assistance for Enhanced Outpatient Sessions (EOS) between clients and the Client Navigation team.
  • Participate in mandatory corporate training, program training, and professional development opportunities.
  • Perform additional duties as assigned.

Qualifications:

  • A High School Diploma is required; a college degree is preferred.
  • At least one year of experience in a substance abuse or mental health setting is preferred.
  • A valid Driver's License is preferred.
  • CPR/BLS certification is preferred.
