QMHP-A Community Services Coordinator

This is an exciting opportunity to join our team as a QMHP-A Community Services Coordinator in the Hampton Roads area. As a QMHP-A, you will work directly with individuals diagnosed with mental illness, providing community-based services and supports to help them achieve their goals and improve their quality of life.
About Our Ideal Candidate:
We are seeking a highly motivated and compassionate individual with a strong background in human services and counseling. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and maintain accurate records.
Main Responsibilities:
- Coordinate scheduling of appointments and services for clients.
- Provide goal-directed training and skill-building to individuals to help them meet their service plan goals.
- Link clients to community-based services and supports, such as housing and employment programs.
- Maintain regular and ongoing contact with referral sources and other involved agencies.
Requirements:
- Bachelor's degree in Human Services or related field.
- Registered with the Virginia Board of Counseling as a QMHP-A.
- A minimum of one continuous year of demonstrated clinical full-time experience.
