POSITION SUMMARY
This role involves conducting initial assessments of clients, formulating the Service Plan/Plan of Care, and executing supervisory visits at clients' residences.

FLSA STATUS: Exempt

JOB RELATIONSHIPS

  • Reports to: RN Manager
  • Supervises: LPN, HHA, CNA

KEY RESPONSIBILITIES
The essential duties for this position encompass the following tasks. Additional responsibilities may be assigned as necessary:

  • Oversee daily schedules and client assignments
  • Handle phone inquiries and conduct client intakes, including after-hours referrals
  • Perform initial and ongoing in-home evaluations, facilitate caregiver introductions, and coordinate client services
  • Supervise, train, and evaluate caregiver performance as needed
  • Complete visit documentation promptly and in accordance with company policy
  • Conduct initial and follow-up visits to prospective and current clients to introduce FirstLight Home Care and enhance relationship development
  • Engage in quality assurance assessments and evaluations of agency services, as requested
  • Stay updated with evolving technology, including software applications
  • Adhere to and promote all company policies and procedures

QUALIFICATIONS
To excel in this role, the individual must effectively perform each essential duty. The following qualifications are required:

  • Graduate of a state-approved nursing program and currently licensed in the relevant state(s)
  • At least one (1) year of nursing experience, preferably in home care
  • Proficient in Microsoft Office and/or Google Suite, along with scheduling software
  • Exemplary organizational, communication, interpersonal, and leadership skills, coupled with a positive and professional demeanor

CERTIFICATIONS, LICENSURE, & REGISTRATION

  • Maintain active state Registered Nurse (RN) licensure
  • Possess a valid driver's license and auto insurance

LANGUAGE SKILLS

  • Ability to read, analyze, and interpret general business publications, professional journals, technical procedures, or governmental regulations
  • Ability to compose reports and business correspondence
  • Ability to effectively present information and respond to inquiries from team members, clients, customers, and the public

REASONING ABILITIES

  • Ability to assess situations and make informed decisions using critical thinking and reasoning
