Amedisys is a leading provider of home health, hospice, and high-acuity care, dedicated to helping patients and families navigate the complex healthcare system. We are seeking a compassionate and skilled RN Hospice Care Specialist to join our team.

Responsibilities
  • Perform initial nursing assessments on new admissions.
  • Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and according to care center policies.
  • Regularly communicate patient progress to the clinical manager, physician, and care team.
  • Make referrals to other hospice care disciplines as needed, or document rationale for not doing so.
  • Perform on-call responsibilities and on-call services to patients/families as assigned.
Qualifications
  • Current RN license, specific to the state(s) you are assigned to work.
  • One year of nursing experience preferred. If less than one year of nursing experience, you can be considered for the RN new graduate program.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
