Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Commonwealth Care Alliance. As a key member of our healthcare team, you will provide high-quality patient care and support in a clinic setting.

Key Responsibilities
  • Manage patient flow through the clinic, obtaining pertinent clinical information and preparing patients for examination.
  • Implement patient treatment plans and therapies, and support the care team in coordinating overall care.
  • Assist patients with dressing, undressing, and positioning, and perform phlebotomy, give immunizations, and obtain specimen samples.
  • Assess patient health needs, establish plans of care, and observe patients for changes in condition.
  • Administer prescribed medications, measure and record vital signs, and provide basic patient care and treatments.
  • Participate in medical supply inventory management, restock supplies, and interact with vendors for deliveries and pickups.
  • Collaborate with Registered Nurses to ensure delivery of quality patient care and maintain accurate and timely documentation of care.
Requirements
  • High School Diploma or equivalent required; Associate degree preferred.
  • Minimum 2 years clinical experience; current LPN license to practice in Massachusetts required.
  • CPR certification and willingness to learn telehealth technologies.
  • Bi- or multi-language skills highly desired.
Working Conditions

This is a standard clinical environment with standard clinical equipment. The physical demands of this job include standing, using hands to handle objects, reaching, talking, and hearing.
