Job Summary:
The Hospice Chaplain is a vital member of our interdisciplinary team, responsible for providing spiritual care services to patients and their families/caregivers in Advanced Hospice. This role requires a compassionate and empathetic individual who can work effectively in an interfaith setting.
Key Responsibilities:
- Conduct spiritual assessments of patients and families/caregivers to identify their unique needs and develop individualized plans of care.
- Provide direct spiritual care to patients and families/caregivers, including counseling, support, and education.
- Collaborate with community chaplains and spiritual counselors to ensure seamless care coordination.
- Maintain accurate records of spiritual care services provided and participate in quality assurance activities.
- Attend patient care conferences and interdisciplinary group meetings to ensure effective communication and collaboration.
- Develop and maintain relationships with community clergy and congregations to support our patients and families.
- Plan and coordinate memorial services and other spiritual events as needed.
- Supervise spiritual care volunteers and provide education and support to the interdisciplinary team on spiritual care.
Requirements:
- Master's degree in Divinity or a related field from an accredited institution.
- Experience working with death and dying, individuals, and families.
- Comfort working in an interfaith setting.
- Knowledge of and commitment to hospice philosophy of care.
- Valid driver's license and reliable transportation.
Skills and Qualities:
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Strong networking and relationship-building skills.
