Job Title: Registered Nurse/Field Supervisor

At Home Helpers Home Care, we are seeking a compassionate and skilled Registered Nurse/Field Supervisor to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care and supervision to our clients in their homes.

Responsibilities:
  • Coordinate total patient care by monitoring client conditions, promoting sound preventive practices, and coordinating services.
  • Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis.
  • Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for home care patients.
  • Consult with the attending physician concerning alterations of Patient Care Plans, checking with the appropriate supervisor and making changes as appropriate.
  • Conduct initial and ongoing comprehensive assessments of the client's needs.
  • Coordinate patient services.
  • Submit clinical notes no less often than weekly, and progress notes and other clinical record forms outlining the services rendered as indicated.
  • Participate in case conferences, discuss with the supervisor problems concerning the patients and how they may best be handled.
  • Discuss with the appropriate supervisor the need for the involvement of other members of the health team.
  • Cooperate with other agencies to provide continuity of care.
  • Continually strive to improve nursing skills by attending in-service education, formal education, workshops, and conferences.
  • Adhere to all Federal, state, and accreditation requirements, including Medicare and Medicaid regulations.
Requirements:
  • Must be a graduate of an accredited college or university.
  • Must be licensed as a Registered Nurse in the State.
  • Must have a minimum of one year of experience in a home health agency or in a hospital setting.
  • Must have knowledge of Medicare and Medicaid guidelines.
  • Should be skillful in organization and in the principles and techniques of professional nursing and required documentation.
  • Should be skillful in organization and in the principles of time management and have knowledge of management processes.
  • Must be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staff.
Compensation:

$45.00 per hour
