Physical Therapist Home Health Hospice Short Hour

This role is designed for Physical Therapists who require flexibility in their schedules and are willing to make home visits at any given time. As a Home Health Physical Therapist, you will provide independent, age-appropriate physical therapy to patients in their place of residence, teaching them and their caregivers preventative and rehabilitative procedures. You will develop and coordinate the plan of care, providing case management and ensuring that patients receive the necessary support to achieve their goals.
Essential Responsibilities:- Evaluate patients and interpret evaluation findings to determine the nature and extent of dysfunction, taking into account adaptive equipment needs and Activities of Daily Living.
- Establish treatment goals and plans treatment to achieve established goals, including adaptive equipment as necessary.
- Initiate discharge planning for physical therapy and implement the plan of care through direct treatment of the patient, delegating tasks to supportive personnel as needed.
- Provide advice and education to patients and their families, regularly reassessing clinical signs and symptoms to determine the effectiveness of treatment and progress towards goals.
- Assess for adaptive equipment for Activities of Daily Living, based on clinical guidelines, and coordinate with certified equipment vendors as necessary.
- Participate in educational and clinical research activities in physical therapy, as well as specialized patient care clinics outside the department.
- Assist in the development, implementation, and review of departmental patient care programs, performing other duties as needed in staff education, departmental operations, and departmental projects.
- Supervise the work of Home Health Aides according to regulation and agency policy, demonstrating ongoing progress towards meeting the criteria for professional practice.
- Provide accurate, timely documentation of physical therapy patient management, including assessment for adaptive equipment.
- Minimum two (2) years of experience in physical therapy.
- Graduate of an accredited physical therapy curriculum with a bachelors, masters, or doctoral degree.
- Driver's License (California) and Physical Therapist License (California) required at hire.
- National Provider Identifier required at hire.
- Basic Life Support.
- Knowledge of Medicare conditions of participation and OASIS.
- Clinically skilled to detect early warning signs of complications, including medical assessments of skin integrity and mentation.
- Able to differentiate emergent versus non-emergent patient care status.
- Able to look at patients holistically, not just body part or functional ability.
- Competent to remove staples/sutures in the home.
- Commitment to Service Orientation.
- Effective English written and oral communication skills.
- Good interpersonal skills.
- Must be able to work in a Labor/Management Partnership environment.
- Experience in an automated clinical information system.
- Computer literate.
- Experience in assessment of high mobility equipment.
