About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team at American Home Health Care. As a Home Health and Hospice Care Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Assessment and Planning: Conduct comprehensive assessments of patients' physical, functional, psychosocial, and cognitive status to determine their home care needs.
  • Develop and Implement Care Plans: Create individualized care plans that establish measurable goals and incorporate therapeutic, preventive, and rehabilitative nursing actions.
  • Provide Skilled Nursing Care: Deliver skilled nursing care, preventative rehabilitative procedures, and prescribed treatments to patients in their homes.
  • Communicate Effectively: Maintain open and effective communication with patients, families, physicians, and other healthcare team members to ensure seamless care coordination.
  • Document Patient Care: Accurately document patient care, including assessments, care plans, and progress notes, in accordance with agency policies and procedures.
Requirements
  • Current RN License in California: Possess a current and active Registered Nurse license in the State of California.
  • Experience in Home Health or Hospice Care: Have a minimum of 1 year of experience as a professional nurse in home health or hospice care within the last three years.
  • CPR Certification: Hold current CPR certification.
Compensation and Benefits
