Job Description

**Job Title:** Home Health Aide

**Job Summary:** We are seeking a compassionate and skilled Home Health Aide to join our team at Grace and Glory Hospice. As a Home Health Aide, you will provide personalized care to patients in their homes, working under the direction of a registered nurse.

Responsibilities:
  • **Personal Care:** Provide personal care services to patients, including bathing, dressing, toileting, feeding, and other activities of daily living, as outlined in the patient's Plan of Care.
  • **Documentation:** Accurately document care provided to patients in our online system, HospiceMD, in accordance with agency policy.
  • **Communication:** Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • **Equipment Operation:** Utilize durable medical equipment and other basic office and medical equipment as needed.
  • **Team Collaboration:** Participate in team and staff conferences to foster a cooperative effort among personnel and ensure effective patient care.
  • **Patient Safety:** Monitor patients' skin for signs of breakdown and report any concerns to the Case Manager.
  • **Respite Care:** Provide respite care to patients' families and caregivers as needed.
  • **Quality Assurance:** Participate in Quality Assurance and Performance Improvement (QAPI) activities to ensure high-quality patient care.
Requirements:
  • **Certification:** Home Health Aide Certification
  • **CPR Certification:** Current CPR Certification
  • **Physical Form:** Physical Form signed by a Doctor (provided by the agency)
  • **TB Test and Immunizations:** Recent 2-Step TB Test and Immunizations or Titers (checks for previous vaccinations)
  • **Identification:** Two forms of identification (driver's license, social security card, passport)
  • **Auto Insurance:** Auto Insurance
  • **References:** Two professional references
  • **Communication Skills:** Good means of communication
  • **Transportation:** Reliable transportation
