About the Position:
Nirvana Healthcare Management Services is seeking an Enrollment Coordinator to join our team. This role is ideal for individuals with experience in healthcare administration, patient services, or case management who are passionate about delivering high-quality patient care and ensuring seamless transitions into healthcare programs.

Duties and Responsibilities:

  • Enrollment Process Oversight: Coordinate and oversee the entire enrollment process for patients into chronic care management (CCM), behavioral health integration (BHI), principal care management (PCM), and other healthcare programs.
  • Patient Communication and Support: Serve as the primary point of contact for patients throughout the enrollment process, addressing any questions or concerns regarding program eligibility and services.
  • Record Keeping and Data Entry: Accurately input patient data into the Clinii system or other relevant databases and maintain accurate and up-to-date records of patient data.

Qualifications and Requirements:

  • Education: High school diploma or equivalent required; associate's degree or higher preferred.
  • Experience: Minimum 2 years of experience in healthcare administration, patient services, or case management.

Compensation and Benefits:

  • Salary: $60,000 - $70,000 per year, depending on experience.
