About Our Opportunity

We are seeking a highly skilled and experienced RN to join our team as a Healthcare Transfer Coordinator. This role plays a critical part in ensuring the smooth transfer of patients between healthcare facilities, utilizing their clinical expertise and knowledge to facilitate effective patient care.

Job Summary
  • Estimated Salary: $83,000 - $110,000 per year, based on experience and location

In this dynamic and challenging role, you will work closely with multidisciplinary teams to coordinate patient transfers, resolving any issues that may arise along the way. Your excellent communication and interpersonal skills will enable you to build strong relationships with referring physicians, nurses, and hospital staff, ensuring seamless patient care.

Responsibilities
  1. Coordinate patient transfers between healthcare facilities, utilizing clinical expertise to match patients with the most suitable destinations.
  2. Collaborate with multidisciplinary teams to resolve any issues related to patient transfers, ensuring timely and effective resolution.
  3. Build and maintain strong relationships with referring physicians, nurses, and hospital staff, fostering open communication and trust.
  4. Stay up-to-date with regulatory requirements, such as EMTALA, HCA Ethics and Compliance policies, and quality initiatives, to ensure compliance and optimal patient outcomes.
  5. Participate in daily bed management meetings to effectively communicate resources and issues, driving improved patient throughput and safety.
Requirements
  • Graduate of an accredited nursing program required; Bachelor's degree in Nursing preferred.
  • Minimum of 3 years' related clinical experience in an acute care setting (ED or Critical Care preferred).
  • RN License in the states where the HCA facilities for that Division are located – multi-state license required if Transfer Center covers more than a single state.
